Homeowners

A signed and notarized Affidavit of Residency, with one of the following:

• Copy of Deed

• Record of most recent mortgage payment

• Copy of recent property tax bill

• Copy of settlement statement

• Fully signed and executed Purchase and Sale Agreement, provided occupancy occurs within 30 days of enrollment, documentation of financing, if applicable

Homeowners & Renters

Provide two from the following list of bills dates within the past 45 days OR a recent statement of service showing the service and connection date:

• Cable/Satellite TV bill

• Internet bill

• Electric bill

• Gas bill

• Home telephone (cell phone bill is not acceptable)

• Car insurance bill

• Home/Renters insurance bill

Homeowners & Renters

Provide evidence from one of the following:

• Valid government-issued photo identification (MA driver's license, MA photo ID, Passport, Other government-issued ID.

• W-2 form dates within dated within the past 12 months, showing current address

• Payroll stub or bank statement issued within the last 45 days, showing current address

Renters

• A signed and notarized Landlord Living Agreement form, copy or current lease, and most recent record of payment

OR

• Tenant at Will form

OR

• Section 8 Agreement
