Registry of Motor Vehicles Request for Permit or License

If you are in need of an address verification to obtain your permit or license, please fill out the form. Please note, we do not provide transcripts to the RMV. We provide a print out of the address on file and a letter stating you are a student at AHS. Requests must be made at least 3 days prior to your appointment for a permit and/or license. Please be mindful of school holidays, vacations and summer hours. If your address has changed, please the process for updating your address.
