[ARCHIVED] End of Year Medicine Pick-Up

Please make arrangements to pick up your child’s medication and or supplies at the Andover High Clinic by 3pm on June 16th.  Any medication not picked up will be discarded. We are here daily from 08:00 – 3:20 pm.

 

There are some medications that we are allowing the students to pick up. Call us to inquire, if you are interested in this option.

 

If your child will need medication for the 2022-2023 school year, a Medication Administration Consent Form must be completed for each medication.  This includes any daily medication, inhalers, epi-pens, insulin, lactose medications, etc.

 

This form may be found on the new nursing website under the icon for Andover High School and in the forms section.  This form must be filled out by your child’s physician as well as the parent/guardian. You may have the order faxed to the clinic if you prefer and our fax number is listed on our web page.  Please bring the completed form with the labeled medication on the first day of school. We will be available on Monday & Tuesday August 29th and 30th, but have several meetings so call us first.

 

You can also find our standing order for analgesics which must be completed every school year. We stock both acetaminophen and ibuprofen. If your child needs liquid you may provide their own for the year.

 

All prescription medication must be in a labeled prescription bottle.  All non-prescription medication must be in a new, sealed bottle or container.
