[ARCHIVED] COVID Vaccination Program

We are excited to announce the opportunity to provide COVID vaccine for students 16 years and older during the school day.

Andover Public Schools, Andover Board of Health and Pelmeds Community Pharmacy will be collaborating in this venture to provide COVID vaccine to our students.

Insurance information will be requested, however, lack of insurance would not prevent a student from receiving the vaccine. Students must be 16 years and over to receive vaccine and parental consent is required.

We will be offering the Pfizer vaccine, which is a 2 dose series. Therefore, the first dose would be given during the week of May 10th and the 2nd dose will be administered 3 weeks later, during the week of May 31st.

Specifically, these initial vaccine clinics will be held at the Andover High School on

  • Tuesday May 11th for Cohort A
  • Friday May 14th for Cohort B

Students must be in attendance within the building in order to participate in this program and students must be pre-registered to receive vaccine.

Print and complete the COVID Vaccine Consent Form.  Return the completed and signed consent form along with a copy of your insurance card to the Andover High School Nurse by Monday, May 10th
